How to Replace a Lost TIN ID

TIN IDs are damaged or lost for a variety of reasons, so do not panic. A TIN ID replacement card can be obtained from the BIR in the following manner.

Ensure that the following requirements are met for replacing a TIN ID that has been lost or damaged:

  • The BIR Form 1905 (Registration Information Update Form) and the BIR Form 0605 (Payment Form) have been completed.
  • For lost TIN ID: Loss affidavit notarized
  • For damaged TIN ID: The old ID card and a photocopy
  • Certificate of marriage and old TIN card (if applicable)

Bring your TIN ID requirements to your RDO once you have completed them. If you don’t have the forms you need, you can download them or get them from the RDO.

  • Complete the BIR Form 1905. The Replacement of Taxpayer Identification Number (TIN) Card under Part II must be Xed.
  • The Lost/Damaged box should be marked as lost or damaged under Reason/Details.
  • Please submit your completed BIR Form 1905 as well as other requirements at the Certificate of Registration (COR) Update window.
  • The BIR Form 0605 should be filled out and verified at the Payment window.
  • To replace a lost or damaged card, visit the bank that is your RDO’s agent. Before paying the fee, request a BIR payment slip. You will need to have your Form 0605 machine-verified by the bank.
  • Your machine-verified Form 0605 and receipt should be submitted to the COR Update window after payment has been received.
  • Please wait for your new card to arrive.

The information on your card will also need to be updated through this process. Please fill out the BIR Form 1905 and mark the appropriate reason with an X if you need to update your civil status or correct a misspelling.

TIN ID Fees

A TIN ID does not require any payment, but a registration fee of 500 PHP is required if you just want a TIN.

TIN IDs are only required when you need a replacement because you lost or damaged your TIN card, and they are only 100 PHP.

You may also encounter people claiming they can process your TIN application or ID for a fee; you will see them on social media or at BIR offices.

You don’t want to be misled by these people, as they will likely be fraudsters. The BIR does not allow third parties to process TINs and ID cards.
